Superbills

Upon request, a superbill can be provided for patients seeking possible reimbursement through their insurance carrier. Reimbursement is not guaranteed and depends on your individual plan’s out-of-network benefits.

Payment is due at the time of service. We accept major credit and debit cards.

A $50 deposit is required to reserve new patient appointments and will be applied toward the cost of the initial evaluation.

We kindly request at least 24 hours’ notice for cancellations or rescheduling. Missed appointments and late cancellations may be subject to a fee.
